I just had a friend pass away from there recently. I donated his belongings to the nursing home, because half of his clothes and shoes were already misplaced (as a worker told me) I actually believe they were stolen, but that's besides the point. I asked where his jewelry was,as I had seen it on him the day before while I was visiting. First answer I got, was he didn't wear jewelry. Lol 😆 sure he did. Then they told me a family member must of took them,well I was his POA and when I told them that,the informed me the funeral home must of took it.. well thankfully they got it right, because I called funeral home and they did have it...
My only fear now, is my brother in law is in there,for rehab. Good thing he has his right mind because I'd be really pissed if something happened to him. Actually his second day there,my sister went to visit him,n he was sitting in shitty pants for about two hours,n the answer she got was,he had to wait his turn, because they were short handed. Seems to me,if you don't have people to take care of patients,don't take new ones on.

I went into the rehab side of Liberty Healthcare which is part of the Windsor family of healthcare.  First off when I got to the facility it was Monday evening about8:00PM. I felt like I was bothering the staff on the rehab floor. I was just put in bed. I was not shown how to work the bed which by the way was on its last legs. I wasn’t told anything. After a day time nurse told me that she felt like the nurses were in my room to much giving meds. Mind you I had just had a rod put in my right leg from my knee to my ankle on 7/5/2025
The hospital told me to ask for the pain meds every 4 hours to stay up on the pain. This was 7/7/2025 so 2 days after surgery.
So after she told me that I told her I was going home that day because of some of the care I received and she was like I hope it wasn’t something I said. Like excuse me it was everything you just said. Jim from PT was the one who talked me into staying until Friday the 10th of July I believe.
Friday came I told the nurse I was leaving at 10:30AM when my family arrived. She wizzed by my room and said I need to print out your discharge paperwork. I told her again I was outs there at 10:30 AM again she just ignored me and walked away. It’s truly not the nurses or aides fault. They are over worked and under paid. They need 2aides and 2nurses to the rehab floor at all times.
PT I cannot say enough about them Jim is the one who talked me into staying until Friday
I went there because it was close to home and my girlfriend of mine her sister was in there about a year ago and received excellent care.
Then after I left the same muse that wizzed by me called my daughter her name was Ashley and left a message for my daughter to call her my daughter did but got a voicemail and just hung up. And Tawny called my husband and left a message and my husband called her back and got voicemail and he did leave a message but to my knowledge she never called him back
It’s sad to see what health care has come to

My husband has been a patient here two times for rehab.  The rehab is top notch. Therapist go above and beyond.  The staff are super friendly.  Kayla Talley went above and beyond to assist in my husbands care.  Kay that runs the kitchen also took the time to meet with my husband on his likes and dislikes along with his nourishment needs. We will only choose this facility if we ever are in need of rehab. Thank you to all.

If I could give it a negative or zero I would.  AWFUL.AWFUL.AWFUL.  My mom was sent there after falling and breaking her tailbone.  She was suppose to be there for rehab and was in a room with someone who had been there 3 years, don’t think that’s rehab.  After the hospital transferred her there I arrived about 15 minutes later.  My mom is 100 years old, and very with it, but her hearing is awful and her hearing aides were in for repair.  My sister and I waited 3 hours for someone to come and assess her, never came.   We knew that she would never be able to hear the questions being asked. It was 5P.M. By now and dinner was being served, sill no one.  A nurse was passing meds and came in her room, talked to the roommate and left, never even acknowledged us.  We packed up my mom’s belongings, put her in her wheelchair, and left.  We walked past a dining room and nurses station and not one person noticed us!  REALLY!!  On our way home we received a call from the administrator, I believe.  One of the secretaries saw us leave.  I explained to her why and she said her nurse said she had assessed my mom around 2:15, I was sitting there and no one came in to talk to us.  Not one person came in to introduce themselves, ask about dinner, or even acknowledge that my mom was a new patient.  My sister and I would not have felt comfortable leaving my mom there by herself.  If this was the treatment she was getting while family was there can’t imagine what would happen if she was there alone.  We left and went to Canfield Windsor, had a great experience there.

I am extremely disappointed in the lack of communication and coordination at this facility. Family members are often left in the dark, with little to no information on the care process or next steps. During my mother’s four-week stay, we received only one care team meeting—and that was after multiple requests. There were no regular updates on her medication or treatment plan, and any questions we had were either unanswered or inadequately addressed.

The facility appears significantly understaffed, resulting in a concerning lack of oversight. On most days, it feels as though the place is scarcely attended to, and communication between nurses and aides is virtually nonexistent. This leaves families feeling uninformed and anxious about the quality of care their loved ones are receiving.

The admission process itself was disorganized and impersonal. Upon arrival, there was no tour or introduction; my mother was transferred directly from the hospital to the facility without any guidance or familiarization. No staff member introduced themselves, leaving us to navigate a confusing environment on our own. Throughout the entire stay, it remained difficult to find anyone who could explain procedures, answer questions, or assist in understanding the admission process. This lack of orientation created an overwhelming and frustrating experience.

The transportation service is similarly unreliable. While we were assured that doctor appointments were scheduled, these were often overlooked, or transportation arrangements fell through entirely. On one occasion, my mother was left waiting for over three hours at an appointment without support—an unacceptable lapse in care.

The discharge planning was equally disastrous. I was shocked by the lack of follow-through and coordination. Remarkably, I received a letter a week after my mother’s discharge, informing us of her discharge date—something we had already endured without proper communication. A specific social worker, Anna, consistently provided misinformation to both our family and the assisted living center, making false assurances throughout the process. The disorganization caused unnecessary stress and anxiety, even bringing my mother to tears as she waited hours past the promised discharge time.

Most concerningly, I learned of a tragic incident involving a resident on the nursing home side of this facility. According to a resident from the healthcare center, a funeral home representative, after waiting 30 minutes to locate staff, resorted to asking another resident to sign for the deceased because no staff could be found. This disturbing lapse in responsibility and oversight speaks volumes about the facility’s staffing and management issues.

In summary, the lack of communication, understaffing, poor coordination, and alarming incidents raise serious concerns about the level of care provided at this facility. I strongly urge the management to make immediate improvements to ensure the safety and dignity of all residents.
