FORMAL COMPLAINT – ELDER NEGLECT AND SUBSTANDARD CARE
I am submitting this formal complaint to report serious and ongoing neglect, unsafe conditions, and substandard care at this facility. The treatment I have personally witnessed raises grave concerns regarding patient safety, dignity, and compliance with elder care regulations.
On multiple occasions, I have observed staff demonstrate a lack of basic compassion and professionalism toward residents. Most concerning, I witnessed staff physically drag a patient in a wheelchair after he mistakenly went to the wrong room. This conduct is not only inappropriate but may constitute physical abuse.
I have also observed systemic failures in nutritional care. Patient meal trays routinely sit for over an hour before being delivered, resulting in food that is cold and unappetizing by the time it reaches residents. The food provided is excessively greasy and nutritionally poor, and in several instances, staff removed the trays approximately 10 minutes after delivery, depriving patients of adequate time to eat. As a result, many residents are effectively left without proper meals.
Additionally, I personally witnessed staff prepare and provide a sugary drink to my mother-in-law despite her being diabetic, demonstrating a dangerous disregard for documented medical conditions and dietary restrictions.
Staff responsiveness is also a serious concern. When my husband approached the nurses’ station to ask a question regarding patient care, the nurse remained on her personal phone and failed to acknowledge or respond to him in any manner.
The neglect escalated to a critical level on Saturday morning. My mother-in-law was transported back to the facility early that day. When we arrived at approximately 10:00 a.m., she was found lying in a soaked bed with a saturated adult brief, indicating she had been left unattended for an extended period. This is a clear violation of basic care standards and places patients at risk for skin breakdown, infection, and emotional distress.
These incidents demonstrate a pattern of neglect, including but not limited to failure to provide timely and appropriate nutrition, disregard for medically required diets, lack of basic hygiene and incontinence care, failure to supervise and protect vulnerable residents, and unprofessional and dismissive staff behavior.
Elderly and medically vulnerable individuals are entitled to safe, dignified, and humane care. What I have witnessed falls far below acceptable standards and may constitute violations of state and federal elder care regulations.
I am requesting an immediate investigation, corrective action, and accountability for this facility. Families should not have to fear that their loved ones are being neglected or mistreated while under professional care.

I cannot say enough wonderful things about this nursing home. My mother lives here, and the level of care she receives is truly exceptional. The staff is consistently warm, patient, and attentive — you can tell they genuinely care about the residents and treat them with dignity and kindness.

The environment is clean, calm, and well-maintained, which gives our family so much peace of mind. Any time we’ve had questions or needed help with something, the team has been incredibly supportive and responsive. It’s clear they go above and beyond to make sure residents feel comfortable, safe, and respected.

Knowing my mom is in such capable and compassionate hands means everything to us. I’m deeply grateful for the entire staff and highly recommend this place to anyone looking for quality care for a loved one.

Toured facility with Amanda from admissions for rehab placement for my mother.  Overall facility was cleaned and well maintained. Compared to other facilities in the area, I liked that this one was well kept and right across from Christ Hospital. Definitely will be having the hospital send over referral for admission. Thanks Amanda for answering all my questions.

**Admissions: 4.5** (The staff was always willing to listen to our needs and took the time to help educate us.)  Amanda was fantastic and always took the time to speak with me and answer any questions

**Social Workers: 4.5** (Anytime you needed them, they were there and did their best to get everything in order.)  Crystal T. And Anthony did an amazing job as on educating me

**Nursing: 4** (They have always given us time and answered any questions we might have. I’m amazed they can manage to care for 15 to 18 patients each shift.)  Nurse Cynthia is 5star nurse in my book

**Nursing Assistants: 4.3** (They do a great job despite taking care of 15 to 18 patients each shift. Imagine being responsible for changing, showering, and feeding so many patients. It's a thankless job, and they get it done.)  Nursing Assistants Latoya is 5-stars and Balinda 4.5. I felt my  mother was in good hands with those ladies.

**Visiting Doctors: 2.5**

**Food: 3.0** (With so many people having dietary restrictions, there’s only so much that can be done.)

**Physical Therapy: 4.5** (They aren’t afraid to push you, but you get out of it what you put in.)

**Nutritionist: 4** (Friendly and takes the time to answer questions.)

**Speech Therapist: 4.5** (Friendly and takes the time to answer questions.)

Scheduling transposition 4.5 very detailed and made sure we understood instructions and patient with us.

We've been in and out of Warren Barr three times, from March to August 2024. My brother and sister were at Warren Barr seven days a week, from 8 am to 8 pm, so I can provide an accurate account of what the place is really like. I looked at the reviews online before bringing my mother to Warren Barr, and I was initially intimidated. However, I decided to visit the facility myself to see what it was like, and I think the reviews are unfair. We live in a society where people expect a steak dinner at McDonald's prices. Many complain instead of getting involved. The staff at Warren Barr is a hardworking group that does not receive enough recognition for their efforts.

Here are some key points about Warren Barr: my mother was taken there for their post-hospital rehabilitation services. This is a place where stable patients go for therapy. These facilities are not intended to cater to your every whim. Nursing staff provides care differently than in a hospital, where a nurse typically cares for 3 to 5 patients, while nurses in a nursing care facility care for 14 to 16 patients. When you first arrive, not much can be done other than processing the patient's discharge paperwork, so the nurse can enter it into their system and communicate with the local pharmacy for necessary medications. You might wait three to four hours before you truly feel like you are receiving service.

I have spoken with Warren Barr about the importance of educating families upon arrival so they know what to expect. If they do not communicate with families, it can lead to feelings of neglect. Acknowledging family members goes a long way in alleviating stress during an already challenging time. Some reviews mention that the facility looks outdated or worn down. However, it's essential to understand that this is a 24/7 operation, and the constant traffic will inevitably take a toll on the premises. Nothing in life is perfect, and while I may need to send my mother back to a nursing care facility, if I do, I will choose Warren Barr again.

If you have family in a facility like this, please step up and help. Your family took care of you when you were helpless, and now it is our turn to do the same. That’s the circle of life.

I would give this place negative ten stars if I could! They don't care about their patients and push people out when they can't even get out of their wheelchair on their own! Their communication skills are nonexistent. The social worker Anthony is ignorant and rude. Had to call over six times to even get a reply. He basically sure said insurance had approved release, but had no idea how long she had even been there and tried to tell us she would be charged $400 a day after release date so we shouldn't bother filing an appeal. He had no idea how she would be being cared for when she was released and just kept reiterating $400 a day if she didn't leave. Whoever does the therapy there...you obviously are very unskilled! I am so pissed off at this place, I can bet there will be a fall within the first week of release and I will be going after them! Take note Warren Barr!
